Well, I've never looked at the script files for Tsukihime before, but I'm guessing that it uses a global variable (carries over after you beat the game). Most likely, you will be able to get all of the routes...unless of course you are lacking some random variable that may have been available at some point before your save.

He means that the global variable makes the route available, but before that point there may be some flags you need to get to actually get the end..

i.e: The choice you've saved just before may not be the only choice you need to make to get the end, and some of those choices may be before that scene.

No you cannot use the same save as some peoples routes are entirely different and do not include the dream. The only exception to that is possibly Arcueid and that girl with glasses who you see fixing stakes at the back entrance of the school near start of game. (i forgot her name, she wasn't really memorable)
